• Other Certifications/Specialty certifications requirements: CCM (Certified Case Manager), ACM (Accredited Case Manager), C-SWCM (Certified Social Work Case Manager), CBHCM (Certified Behavioral Health Case Manager); CCM preferred but not required

  • Experience or 'Must have' skills: Must be RN Case manager with at least 2 years Acute Care Management experience with previous experience in large academic Medical Center hospital
  • UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options

      • Schedule Information
      • Shifts & Scheduling: M-F 8-430p
      • Weekend Requirements: None
      • Holiday Requirements: Rotated as needed
      • Department Specific Orientation: 2 weeks (will include general facility orientation)
      • Will work alongside CM team so will never be working alone
        • Unit Specific Information
        • Case load/# patients per day/consults per day: 15-20
        • Do you require InterQual criteria or Milliman experience: No, but need to have general understanding of discharge readiness
        • What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Will cover all Inpatient units throughout the hospital
        • Social Work team consists of 15 full-time and 17 part-time Social workers
        • Scrub Color/Dress Code: Any color scrubs or business professional attire
          • Additional Details
          • BLS; START DATE: 11/11
          • 50 mile radius rule
          • Fingerprint requirement can be done remotely (takes approx 10-14 DAYS) or in person in Jackson, MS 1 week prior to assignment
          • EMR: EPIC; Will use naviHealth to send referrals and plan to switch to EPIC Care Management over the next year
          • Common types of patients & job duties: Medical discharge could be some social discharges
